The Community Staff Nurse position at Dorset HealthCare University NHS Foundation Trust in Gillingham offers a part-time role requiring 22.5 hours per week. The successful candidate will deliver high-quality, holistic nursing care to patients in their homes, supported by a team of community nursing professionals. Responsibilities include conducting nursing assessments, developing care plans, and coordinating with various health and social care providers to ensure comprehensive patient care. The role mandates excellent communication skills and will offer professional development opportunities to enhance clinical competencies.

Candidates must be registered nurses and are ineligible if requiring Skilled Worker sponsorship to work in the UK. An Enhanced DBS check is necessary, which will be sponsored by the Trust. The position aims to foster a diverse, skilled workforce and provides support for applicants in the form of employability services, including assistance with job applications and interviews.

We currently have a band 5 vacancy in the Gillingham District Nursing team. The hours available are between 22.5 per week. Excellent communication skills are required when caring for patients in their own home. Any clinical skills required for this post will be supported.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Work as a registered nurse as and when required with community nursing teams, providing evidence based, high quality, holistic nursing care for patients in their own homes.

Carrying out nursing assessments, treatment and care and liaising with other agencies as appropriate.

To be developed and supported by Dorset healthcare with training and development to support a registered nurse with caring for patients in the community setting.

To assess, plan, deliver and evaluate nursing care for patients to meet their specific needs.

To assess the needs of patients in a variety of settings and formulate a plan of care. This may involve liaison with carers, other health professionals, GP's, consultants, hospital nurses, social workers, hospice staff, specialist nurses, voluntary and statutory workers.

To provide evidence based nursing care to a defined patient caseload with the support of the District Nurse.
