The role of a Mental Health Practitioner at Hampshire and Isle of Wight Healthcare NHS Foundation Trust in Gosport involves providing intensive home-based treatment as an alternative to hospital admission, working within a 24-hour acute care pathway. The practitioner will be responsible for assessing, implementing, and evaluating the care needs of service users in their home environment, coordinating closely with inpatient units and Community Treatment Teams to facilitate smooth transitions and safe discharges.

Additionally, this position requires the practitioner to lead a flexible, team-based caseload, mentor and supervise both qualified and unqualified staff, and engage in shift work. The ideal candidate will be proactive in promoting social inclusion, independence, and recovery among service users, while also participating in ward rounds and professional meetings to identify individuals suitable for the Hospital at Home Service.
