The Sheffield Churches Council for Community Care is seeking a compassionate and self-motivated Support Worker for their Discharge Support and Admission Prevention role. This position involves providing vital assistance to older individuals, either transitioning from hospital or at risk of admission, across various community schemes. Key responsibilities include coordinating support services, facilitating safe hospital discharges, and offering practical help with daily tasks such as shopping, transportation, and minor home adjustments, all aimed at enhancing the quality of life and independence of vulnerable adults.
