Jubilee Medical Centre in Liverpool is looking for a motivated Clinical Pharmacist to join their team within the North Liverpool Primary Care Network (PCN). This role involves ensuring the safe and effective use of medications, conducting comprehensive reviews and assessments, and collaborating with healthcare professionals to create personalized medication therapy plans. The successful candidate will also provide patient education on medication use, monitor therapy adjustments, and take part in multidisciplinary meetings to enhance patient outcomes.

As the largest PCN in Liverpool, encompassing 21 practices and serving a population of approximately 150,000, the team includes a substantial pharmacy staff of 14 pharmacists and 9 technicians. The position offers supportive induction training and ongoing educational opportunities to help develop the skills necessary for making a meaningful impact on patient care and improving health outcomes within the community.

We are seeking a highly motivated Clinical Pharmacist to join our team in North Liverpool PCN. As a Network Pharmacist, you will play a crucial role in ensuring the safe and effective use of medications, providing pharmaceutical care and expertise to patients, and collaborating with healthcare professionals to deliver exceptional patient outcomes. Responsibilities

Conduct comprehensive medication reviews and assessments to identify and resolve medication-related problems for patients.

Collaborate with physicians and other healthcare professionals to develop and implement personalised medication therapy plans.

Provide patient education on medication use, potential side effects, and drug interactions.

Monitor and adjust medication therapy based on patient response and laboratory values.

Conduct medication reconciliation and identify discrepancies to ensure accurate and up-to-date medication lists.

Participate in multidisciplinary meetings to discuss patient care and optimise treatment outcomes.

Provide education sessions for GPs, practice staff, other health professionals and patient groups.

Stay up to date with current literature and guidelines to provide evidence-based pharmaceutical care.
