The Barnsley Healthcare Federation is seeking a Social Prescriber to join their Barnsley Primary Care Network (PCN). This role involves delivering a Social Prescribing Service aimed at enhancing the health and wellbeing of patients across Barnsley. The successful candidate will assess patients' needs, develop personalized care plans, and connect individuals to community resources and support services.

Key responsibilities include:

- Evaluating how community services can meet patients' health and wellbeing needs.
- Co-producing support plans that link patients to community groups and statutory services.
- Providing ongoing personalized support to patients, families, and carers.
- Building trusting relationships and taking a holistic approach to patient care.
- Promoting social prescribing and training others on referral processes.
- Collaborating with local agencies, voluntary organisations, and community groups to strengthen support networks.

The position involves working with various patient demographics, including children, adults, families, and carers, and aims to empower individuals by tackling wider health determinants like social isolation, housing issues, and mental health needs.
