The role of Primary Care Mental Health Practitioner in Denbigh involves working within the community mental health team to provide assessments and brief interventions for individuals experiencing mild to moderate mental health issues. The practitioner will receive referrals from GPs and will collaborate closely with a multi-disciplinary team, including secondary care, specialist services, and third sector agencies. Key responsibilities include managing a caseload, formulating risk management and care plans, and improving relationships with external agencies.

The ideal candidate should be experienced, motivated, and possess excellent communication skills, along with the ability to effectively prioritize and manage their time. Familiarity with third sector agencies is beneficial, and while the ability to speak Welsh is desirable, both Welsh and English speakers are encouraged to apply.
