The Home Care Practitioner role in Cleator Moor with North Cumbria Integrated Care NHS Foundation Trust involves working alongside a community team focused on palliative care and rehabilitation. The primary responsibilities include delivering personal care, catheter care, nutritional support, and end-of-life comfort to patients, all aimed at promoting independence and ensuring safe and timely hospital discharge to prevent readmission.
