The successful candidate will work within a multi-disciplinary team based in Maidstone, covering areas like Tunbridge Wells and Edenbridge, focusing on recovery, social inclusion, and social justice. Responsibilities include conducting assessments, developing support plans, and delivering individual therapeutic interventions. As a Lead Health Care Professional for those with serious mental health difficulties, the role emphasises a person-centered approach and adherence to principles that prioritize the needs and rights of individuals in care.
You will be joining KMPT at an exciting time. The work around the implementation of the Community Health Framework which will enable Social Work to develop a strong profile within secondary care mental health service to improve patient experience and compliment our existing MDT.
As a social worker you will provide a high standard of support and care delivery with regards to social care needs to those service users whose ability to cope with the tasks of living are threatened or impaired by mental health issues, emotional, psychological and social problems.
You will be working in the West Kent Home Treatment Team, located at Priority House, Maidstone. The Team cover the West part of Kent and this includes Maidstone, Tunbridge Wells and Edenbridge..
You will work closely with other agencies and individuals to support service users with a clear focus on recovery, social inclusion and social justice. The role will entail assessment, support planning and the opportunity to provide 1-1 strength based therapeutic interventions.
You will act as Lead Health Care Professional, (where applicable) with individuals who have serious and enduring mental health difficulties.
