Mount Chambers is seeking an experienced practice nurse for a part-time role in Braintree, requiring 25-30 hours per week over three days. The responsibilities include conducting health assessments, administering treatments, performing diagnostic tests, and developing person-centred care plans for patients, particularly those with long-term conditions. The nurse will also manage urgent health concerns, support women's health services, and participate in vaccination programs, wound care, hypertension management, asthma clinics, and health checks for new patients.
This role is integral to ensuring the health and social care needs of patients are met through a comprehensive approach to health promotion and disease management, in line with both national and local policies. The practice nurse will play a vital part in delivering high-quality care and supporting the overall well-being of the patient community.

Mount Chambers has a fantastic opportunity for an experienced practice nurse to join our team on a part-time basis, for 25-30 hours per week on over 3 days.
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
- Meeting the needs of patients presenting for opportunistic wound care.
- Hypertension review and management.
- Running asthma clinics and reviews.
- New patient and health checks.
- Phlebotomy.
