Norfolk Community Health and Care NHS Trust is seeking a Community Assistant Practitioner to join the King's Lynn Community Nursing Team. This role offers an opportunity to make a significant difference in the lives of patients with long-term conditions, palliative care needs, and chronic wounds. The successful candidate will work collaboratively within an integrated team to facilitate early hospital discharges, prevent unnecessary admissions, and support chronic wound management under the supervision of qualified practitioners. The position requires effective communication with patients, carers, and other healthcare professionals to ensure seamless service delivery and coordinated care.

The Community Assistant Practitioner will be responsible for assessing patients, carrying out and monitoring treatments, documenting visits, and maintaining patient records. They must demonstrate sound clinical reasoning, problem-solving skills, and the ability to liaise with various stakeholders, including GPs, social services, and voluntary organisations. Flexibility in working patterns is encouraged, with shifts operating between 08:00 and 20:00, seven days a week. Candidates should be prepared to handle responsibilities involving direct patient care and indirect tasks while adhering to safety and infection control protocols.
