The role of Community Mental Health Practitioner with South West Yorkshire Partnership NHS Trust in Huddersfield involves providing high standards of mental health assessment, treatment, and care to service users to support their wellbeing within the community. This position operates within a community team and emphasises collaborative work with service users and colleagues, as well as coordination of care, workload prioritisation, and maintaining effective communication with various agencies. The practitioner will manage a diverse caseload encompassing both functional and organic mental health issues, with responsibilities including conducting assessments, reviewing care plans, administering depot medication, and contributing to the education of students and newly qualified staff.

Working hours are Monday to Friday, 9 am to 5 pm, with the option for flexible arrangements. The role requires a car driver for travel across Kirklees and encourages professional development through team support. The position necessitates a commitment to high professional standards, maintaining registration, and participating in service development and clinical governance. The team fosters a culture of well-being and support, aiming to enhance the practitioner’s clinical skills in a multi-disciplinary environment.
