The Rehabilitation Assistant / post holder is responsible for the provision of highquality, individualised personal care and rehabilitation programmes to ourpatients on Caterham Dene Ward, reflecting the philosophy of First CommunityHealth and Care.
They will work under indirect supervision of the lead clinician and will managetheir own upskilling caseload and be responsible for maintaining up to datepatient records.
They will carry out delegated patient related activities, utilising knowledge andskills, within assessed competency frameworks and will work collaborativelywithin the team to meet the needs of the patients, following policy andprocedure.
They will enable the smooth running of the Bed Based Care Service throughcompletion of day to day duties and delegated responsibilities.
The post holder will be personally responsible for not undertaking any action whichwould knowingly cause risk to them, others or the company.
The team provides a seven day a week rehabilitation service at Caterham DeneHospital and visit patients own homes to enable safe discharge. The Post Holdermust have a car for personal business use and be able and prepared to drivebetween our Base at Caterham Dene Hospital and other community settings, asrequired, during their working day.

We are dedicated to supporting our patients with their transition through illness and injury and their hospital stay to maximising their rehabilitation potential and enabling a safe discharge plan, usually to home.
