The EAST:MK Primary Care Network is seeking a full-time Clinical Pharmacist to join their practice team in Bradwell Common, working 37.5 hours over a flexible schedule from Monday to Saturday across four surgeries. The role involves collaborating closely with the multidisciplinary team to ensure evidence-based, safe, and cost-effective prescribing practices. The Clinical Pharmacist will support general practice staff with medication inquiries, conduct medicines reconciliation, manage chronic disease areas, and lead structured medication reviews, particularly for patients with complex medication regimens.

Additional responsibilities include improving prescribing practices through clinical audits, interfacing with community and hospital colleagues to address medication-related issues, and assisting with enhanced health initiatives in care homes. The position is integral to providing clinical expertise to patients and practice teams and supports the delivery of the Network Contract Directed Enhanced Service (DES) within the Primary Care Network.
