To support the Lead Tissue Viability Nurse Specialist in providing a nursing service within the specialty of tissue viability, pressure ulcer prevention and management and leg ulcer management by receiving direct referrals.
In conjunction with the Lead Tissue Viability Nurse Specialist to act as a resource for patients, families and staff in general but especially in terms of:
the in-patient pathways relating to wound management
pressure ulcer risk factor assessment, prevention and management
in-patient pathways relating to leg ulcer management, including compression therapy
advice on invasive diagnostic tests: hand-held doppler readings and ABPI measurementsTo manage an agreed caseload, with the support of the Lead Tissue Viability Nurse Specialist, using in-depth knowledge of the patient / client group.
Use clinical judgement to plan, implement and evaluate care for patients with complex physical and psychological problems.To assist the Lead Tissue Viability Nurse Specialist in ensuring the provision of a cost-effective service, demonstrating sound knowledge and high standards of care whilst working within available resources.
To respect patient confidentiality and need for privacy and to act as a patient advocate.
To act as an expert role model in tissue viability nursing for nursing colleagues and all members of the multi-disciplinary team to promote the welfare of the patient.
To ensure accurate, comprehensive and contemporaneous nursing records are maintained using appropriate Trust documentation.
To contribute to the implementation and on-going development of Trust policies and guidelines relating to pressure area management, leg ulcer management and wound management at a local and national network level.
To support the Lead Tissue Viability Nurse Specialist in the day to day effective and efficient management of nursing staff within the department and deputise for him/ her when he/she is away
To support the Lead Tissue Viability Nurse Specialist in activity planning and scheduling duty rotas, annual leave, study leave, monitoring absences and the use of temporary staff.
To support the Lead Tissue Viability Nurse Specialist in maintaining systems for induction, mandatory training and staff education records
Act as an appropriate and effective role model at all times
Provide day to day guidance and support to the colleagues, ward and clinic nursing staff and medical colleagues
Act as an effective mentor and preceptor and support nursing and medical colleagues in the delivery of high quality care
Act as a resource supporting and motivating staff
Keep abreast of and participate in local trust initiatives
Support the Divisional and Trust Strategies to develop patient care by participating as appropriate in their implementation.
In conjunction with theLead Tissue Viability Nurse Specialist deliver an agreed program of education to all Trust Staff as agreed with the Lead Tissue Viability Nurse Specialist.
In conjunction with the Lead Tissue Viability Nurse Specialist review, evaluate and develop the programme
In conjunction with theLead Tissue Viability Nurse Specialist participate in the development of guidelines and measurable competencies for nursing staff in relation to specific wound management skills
To continue professional development using the appropriate Trust frameworks, in collaboration with the Lead Tissue Viability Nurse Specialist.
In collaboration with the Lead Tissue Viability Nurse Specialist assist in the preparation and present audits of Tissue Viability patient activity for the service
In collaboration with the Lead Tissue Viability Nurse Specialist contribute to the production of outcomes in an annual report
In collaboration with the Lead Tissue Viability Nurse Specialist monitor standards of care and assist in the improvement of care.
Collaborate as required with other members of the MDT in research and audit activities
Assist the Lead Tissue Viability Nurse Specialist in the provision of an evidence based service, through regular review of the literature and application of new evidence to practice as agreed and, as appropriate, with members of the multi-professional team
Contribute to nursing research within the Tissue Viability specialty.
Participate in investigations, serious incident investigations and safeguarding meetings particularly in relation to pressure ulcers
All staff have a responsibility to report all clinical and non-clinical incidents promptly and when requested to co-operate with any investigation undertaken

This is an excellent opportunity for an energetic and motivated healthcare practitioner who has a genuine desire to deliver a high standard of care in all aspects of wound care.
The role entails the assessment and management of patients who have pressure ulcers, leg ulcers and other complex wound care needs. The successful applicant must have experience in leg ulcer and pressure ulcer management and existing wound care skills. They will be expected to undertake educational programmes to clinicians and patients.
The potential post holder needs to be a committed and confident individual who is conscientious, dynamic, pro-active and has excellent communication and organisational skills.
- Have an active role in all aspects of wound care management within the Trust, including leg ulcer management, pressure ulcer prevention and management, and complex surgical wounds.
- He/ she will work collaboratively with colleagues in the tissue viability team and other colleagues across the Trust to facilitate high quality individualisedcare to patients
- The post holder will be required to undertake triage duties and manage the service in the absence of the Lead Tissue Viability Nurse Specialist.
- The post holder will support the Lead Tissue Viability Nurse Specialist in monitoring standards of care by clinical audit throughout the Trust and in delivering the Tissue Viability teaching programmes to all professional groups
- He/she is expected to work with minimal close supervision.
