CASELOAD MENTAL HEALTH NHS ROLE MODEL
The Care Coordinator role in the Colchester Specialist Community Mental Health Team at Essex Partnership University NHS Foundation Trust involves managing a caseload of service users by assessing their care needs and coordinating appropriate interventions. Responsibilities include developing and updating person-centered care plans, implementing evidence-based practices, and continually assessing risk while ensuring adherence to the Mental Health Act 1983. The Care Coordinator also facilitates referrals to crisis teams, administers medications, monitors adherence and side effects, and oversees safeguarding investigations, all while providing guidance and support to junior staff.

The position requires the ability to build therapeutic relationships with clients and promote service user and carer involvement in care planning. With the support of a Team Lead, the Care Coordinator is expected to be a dynamic and forward-thinking professional, acting as a resource for the multidisciplinary team while ensuring that all care needs are effectively addressed within the community setting.
