Our Social Prescribing Link Worker will help and support patients by signposting them to sources of support in the community.
Social Prescribing Link Workers play a crucial role in helping people to get the right support, at the right time to help manage a wide range of needs. These may be supported in-house or by signposting to a range of statutory and voluntary sector services. This may include, help with finance, housing, lifestyle, mobility, social and general advice.
The main duties of the role are the following:
Manage your own patient caseload
Receive and self-manage ongoing referrals into the service
Identify needs of patients and support these
Deliver high quality support to patients in a timely fashion
Signpost patients to relevant services and support them to access these services
Maintain accurate data recording within GP clinical systems
Lifestyle and weight management advice
Refer to other services
Attend team meetings and network independently with voluntary and community sector services.

Ensure principles of openness, transparency and candor are observed and upheld
Have an understanding of GDPR and patient confidentiality
Undergo safeguarding training and adhere to practice safeguarding policies
Complete all mandatory training modules
Hambleton North PCN are looking to recruit a Social Prescribing Link Worker, to be based at Stokesley Surgery / Great Ayton Health Centre. Social Prescribers help support patients to access local services helping them meet their non-medical needs. They play a crucial role in helping people to get the right support at the right time. Support offered by the Social Prescribing Service may include support with things such as the management of long-term conditions, housing and benefit needs, social isolation, and any other practical or social support necessary. We would like the successful candidate to also focus on proactively contacting our prediabetic patients and those on the obesity register to help with weight management, giving lifestyle advice and guidance.
This post is a PCN role, but the successful candidate will be employed by Stokesley Surgery on behalf of the PCN.
This position will cover maternity leave and become a job share role with our current Social prescriber, when she returns.
The successful candidate will have a genuine interest in improving patient outcomes, will be able to work independently without direct supervision and will be calm and professional when supporting patients.
We ask that applicants are open to working overtime when necessary and must have a professional approach to work. The successful applicant will be confident, assertive and have excellent communication skills.
All training will be provided.
