The Long Term Conditions and Health/Wellbeing Coach role at Stockport's Bramhall and Cheadle Hulme Primary Care Network (PCN) focuses on empowering patients with long-term health conditions to manage their wellbeing actively. The coach will work closely with patients, their families, and carers to establish personalized health goals, supporting self-management through coaching, education, and community connections.
