Exciting opportunity to support in the development of a new discharge to assess model within Cheshire Place. You will coordinate and facilitate the safe and effective discharge of patients from Macclesfield District General Hospital and provide initial assessments and interventions in the home environment where indicated.

To provide assessments for discharge pathways 1-3.

Working proactively within an integrated health and social care team, to promote formal and informal channels of communication both within the team, ward staff, ICB and wider community as appropriate.


Hours per week: 37.5

To work as a clinical specialist therapist (Physiotherapist or Occupational Therapist) specialising in the older people living with frailty and Discharge to Assess.

This role will deliver clinical therapy assessment and interventions for patients across the acute and community settings within East Cheshire NHS Trust. It is recognised that older people living with frailty can present across a range of specialities and areas, therefore it is expected that the individual work in an integrated way linking in with multiple teams to deliver high quality multi-professional care across East Cheshire NHS Trust.

The role will support the delivery of a revised Discharge to Assess model across Cheshire place.
