The Essex Partnership University NHS Foundation Trust is seeking enthusiastic individuals for the role of Integrated Clinical Assistant within the newly established South East Essex Frailty Virtual Ward. This full-time or part-time position involves collaborating with a multidisciplinary team, including Consultant Geriatricians, Community Nurses, and Therapists, to provide care in patients' homes. The aim of the service is to alleviate pressure on the NHS by reducing the need for inpatient beds and emergency department visits, ensuring patients receive quality clinical care in a safe environment.

The Frailty Virtual Ward operates daily from 8 am to 8 pm, emphasizing the importance of delivering comprehensive healthcare to frail patients in the community.
