We are looking to recruit an enthusiastic and highly motivated Band 3 therapy rehab assistant to our Hertfordshire Community Team in Hemel Hempstead and surrounding area.

The role involves working with patients in their own home to provide rehabilitation services and promote independence. It will involve exercise provision, assessment and provision of equipment to support them at home. You will also be responsible for equipment and mobility aids as well as assisting at times with administrate tasks for the therapists.

Successful applicants will be part of an enthusiastic, patient focused, holistic team caring for patients in their own homes. Our Multi-disciplinary team comprises of Physiotherapists, Occupational Therapists, Nurses, Healthcare and therapy assistants, as well as support from administrators.

We work collaboratively with GPs, acute hospitals, social care, hospices and other third sector organizations. You will have the opportunity to work with patients with a wide range of clinical conditions.

We have a well-established structure for clinical supervision and opportunities for practice development including in-service training and skills courses. This will provide you with fantastic opportunities to enhance your existing knowledge and skills and progress up the pay scale.

This is a part-time post based in the Dacorum Community Team in Hertfordshire.

To be an active member of the clinical rehabilitation team.

To rehabilitate and maximise a patient's potential. To support and enable patients in a range of activities to promote independent living and monitor their wellbeing.

Provide individual therapy treatment under the guidance of physiotherapy and occupational therapy colleagues, in the community setting.

Manage own caseload of patients and maintain professional documentation.

Build and maintain relationships with other local services to facilitate appropriate referrals and high quality healthcare.

Provide guidance and information to patients, health and social care professionals, carers and relatives and other non-professionals.

To deliver appropriate treatment in a variety of settings including the patient's own home, residential and nursing homes and day centres.

Actively participate in Continuing Professional Development and Clinical Supervision to support your own development and ensure clinical practice is maintained at a high standard.
