The role of a Frailty Occupational Therapist at The Hillingdon Hospital NHS Foundation Trust in Uxbridge focuses on supporting a frailty team dedicated to improving patient care and outcomes within various departments, including the Emergency Department and the Frailty Assessment Unit. Candidates will conduct thorough and timely holistic assessments utilizing the Comprehensive Geriatric Assessment framework, deliver targeted therapy interventions, and facilitate safe patient discharges to community support services. Working collaboratively within a multidisciplinary team, the therapist will be responsible for managing their caseload effectively, setting joint goals with patients, and ensuring seamless transitions of care.

This position requires participation in a 7-day service and may involve covering roles within the wider Older Adults Therapy Team. The successful applicant will be expected to uphold professional accountability for their work, manage priorities efficiently, and engage in ongoing communication with both patients and community agencies to optimize care.
