Lewisham & Greenwich NHS Trust's Children's Community Nursing Service is made up of 5 teams, including a Practice Development Nurse. Our Hospital @ Home Team works alongside local acute Trust's to provide early and safe discharge from hospital and hospital admission avoidance for our paediatric population. We are a 7 day a week service and the Team Lead is a vital part of this clinical work.

- The post holder will work as an autonomous expert practitioner with responsibility for leading a team of nurses and provide highly skilled & developed quality nursing care for children in their own homes and other community settings.
- To develop, implement and evaluate a seamless Hospital@Home (H@H) Service, ensuring that patients receive the highest standard of clinical care.
- To work as an autonomous practitioner with responsibility for facilitating early discharge, reduce length of stay and prevent hospital admissions
- To provide high quality, specialist nursing care to children in their own homes
- To provide continuing responsibility for the assessment, development, implementation and evaluation of nursing care for patients.
- To work in partnership with members of the multi -disciplinary team to maintain high quality care & promote practice that is family centred.
- To continue to develop and maintain close links between Community and Hospital based services.
- .To be responsible for providing support to junior team members through developing their skills & completing staff appraisals. They will motivate staff to provide high standards of care by acting as a role model.
- To undertake the organisation and support of student placements & mentorship through their practice placements.
- To undertake and implement research and audit activities to ensure evidence-based practice and best practice guidelines are delivered within the service.

- To initiate appropriate work, accepting referrals and be able to work autonomously with the child and family in the community setting.
- To regularly attend and liaise with acute hospital colleagues to enable timely and safe discharge from hospital and appropriate follow-up in the community setting
- To collect and analyse daily reports from Paediatric Accident and Emergency (A&E)and follow up any children who need further support in the home setting
- To establish contacts with GPs who regularly send children to A&E and promote the service offered in the community
- Recognise and monitor acute exacerbations of illness and initiate appropriate nursing interventions.
- To provide a holistic, autonomous and clinical nursing service as an expert practitioner using high levels of decision making and clinical judgement and evidence based practice. To carry out initial assessments/home visits and plan, implement and evaluate nursing care for all patients for whom he/she has continuing responsibility. Act in partnership with the child and family when assessing a childs needs and planning. To provide advice contributing to the well-being of the patient and participate in health promotion. (eg burns advice). To provide high quality nursing care to meet agreed pathway standards of care.
- To ensure medical needs and instructions are carried out in accordance with nursing and midwifery council guidelines and ensure accurate record keeping. To create reports for specific formal meetings such as child protection case conferences. Ensure confidentiality of information is adhered to at all times.
- To liaise/make and receive referrals from other disciplines and make decisions regarding client treatments/outcomes without direct supervision.
- To carry out specialist procedures in the community settings
- To use clinical judgement and expertise in assessing and implementing change during clinical visits eg assessment of respiratory condition and management of any changes
- To use clinical knowledge and expertise in deciding appropriate administration of drugs, route to be administered and dose of the drug, within a set of defined parameters prescribed by medical colleagues
- To be familiar with practices regarding the management of hazardous/biological substances and follow trust policies regarding their disposal.
- To undertake individual risk assessments to support children and families within their own homes
- To be familiar with safe manual handling practices and implement within the practice setting when both working with clients and service equipment.

To be responsible for team members as their line manager including management of sickness absence, poor performance and recruitment and selection decisions. Must be able to manage a team of nurses including allocation, delegation & supervision of care given according to their knowledge & expertise. Provide training and organise professional development plans, appraisal & clinical supervision for junior team members/carers/other professionals.
Participate in appraisals and objective setting for junior staff to enable them to achieve optimal effectiveness
Direct, support, encourage and advise team members, by the provision of specialist knowledge and advice in the care of the patients.
Act as a role model, demonstrating high standards of care and providing clinical leadership within the team.
To participate in clinical supervision and personal development programmes
Supervise & assess qualified staff members, nurse learners & post-basic students. Work within local and national policies and procedures to ensure that safe working practices are maintained for both patients and staff.
Demonstrate the ability to adapt own behaviour and influence others to manage conflict and improve team dynamics.
To be aware of health & safety issues at work and ensure implementation of health and safety regulations. Cascade to other team members. Implement risk assessment policies and quality assurance. Notify the Trust of any risks associated with H@H work and with the Lead Nurse ensure it is on the risk register for the service
To act for the Community Childrens Matron during absences as requested.
There is a requirement to provide the service 365 days a year and team leaders are expected to take turns in being on call over weekends and public holidays being the named contact point via bleep during these times and supporting & directing staff who are working with them
To manage client caseload and have the ability to practice autonomously.
To raise the profile of H@H Team within other locations and with other professionals.
Create and develop practice protocols locally according to best practice guidelines and in line with Trust Policies. Cascade knowledge to junior team members and act as a role model.
With other senior staff, participate in planning for the future service provision of the service.
Demonstrate effective leadership qualities .
Facilitate a high quality, cost effective service through undertaking research, surveys, audits in connection with own work. Implement findings to improve patient care and ensure compliance with agreed standards.
Utilise research findings to develop new ways of working and disseminate relevant information to staff.
To assist the lead nurse with updating or writing new care plans, policies, guidelines and competencies.
Set, monitor and review standards of care on a regular basis and initiate corrective action plans.

To have studied or be working towards completing the Advanced Nurse Practitioner qualification
To undertake mandatory training as outlined by Lewisham and Greenwich Healthcare Trust
To implement induction programmes for new members of nursing staff. To supervise/assess nurse learners and student
To plan, design & initiate, monitor & evaluate teaching & learning programmes for a variety of professional and voluntary workers including, clients, carers, junior team members, nurse learners and other professional staff when required.
To be pro-active in promoting the health and welfare of clients and families through providing advice and resources that empower families towards a healthier/safer lifestyle.
- To demonstrate accountability in the use and management of all resources including financial.
- Implement new initiatives where possible to run a cost effective service.
- To participate in working parties in order to propose changes to policy, procedure and guidelines within child health.

- To assess care needs and liaise with other professionals in order to assist them to develop a package of care and to provide clinical/professional advice relating to this. To actively work together with other professionals by communicating within the extended multi-disciplinary teams within the community setting. - To attend Childrens Inpatient Ward and A&E on a daily basis to ensure all appropriate referrals are made and to provide seamless care from hospital to the community setting. To be involved in the providing and receiving of delicate and sometimes complex information during these visits. To negotiate communications with families and carers around complex issues regarding management of care in the community, including bereavement support and managing difficult situations.
- To initiate and negotiate communications with families & carers frequently around complex, emotive and contentious issues regarding management of a childs care in the community. Respect the child and familys culture and diversity & communicate in a sensitive manner. Use appropriate resources as required such as use of interpreters.
- Identify and intervene where circumstances contribute to an unsafe environment for patients and staff and bring to the attention of Lead Nurse.
- To be responsible for the demonstrating of safe use of expensive and complex pieces of equipment. To be responsible for ensuring that medical supplies are ordered from an agreed list in order that the team remain within budget.
- To assess patients needs and initiate all requests for nursing aids or equipment. To be responsible for the training of their correct use.
- There is a requirement for the post-holder to be flexible with their working hours and cover over a weekend or Public Holiday
- To act as a resource and role model to junior staff members & other professional staff
- Take an active role in risk assessment, supporting implementation of strategies to minimise risk. Ensure incidents and near misses are reported, through promoting a no blame culture.
- There is a requirement for the post-holder to be IT literate as there is a regular requirement to use computer software to create reports. The team use RIO for all of their client notes.
- To participate in teaching Nurse Learners and all other students as appropriate who visit.
- To frequently market and update service with acute colleagues, attending and teaching at doctor/nurse training sessions on a regular basis
- To provide a high standard of professional conduct and nursing care at all times in accordance with the NMC.
- To attend relevant meetings, courses, seminars and keep professionally up-to-date.
