The Afan Local Cluster Collaborative Lead is a pivotal role within the Swansea Bay University Health Board, overseeing a diverse partnership ecosystem that includes Social Services, health professionals, and primary care contractors, serving a population of over 50,000. The Cluster Lead will provide strong and inspiring leadership to unite stakeholders, translate national health strategies into actionable plans, and drive improvements in community health service delivery. Key responsibilities include remodeling care pathways, supporting the development of effective governance systems, chairing LCC meetings, and managing resources in alignment with Health Board strategic plans.

This part-time position (0.2 WTE) calls for candidates with local knowledge of the Afan area and its demographics, as well as a commitment to fostering collaborative approaches to health improvement. The ideal candidate will demonstrate leadership skills while working closely with Health Board Officers and LCC partners to enhance population health outcomes and lead the LCC in its annual planning cycle. Proficiency in Welsh is desirable, though not mandatory.
