The Care Coordinator role with the Windsor Primary Care Network involves working within a multidisciplinary healthcare team to enhance personalized and coordinated health services for a diverse patient population of 70,000 in Windsor, Berkshire. The successful candidate will focus on proactively identifying and supporting individuals, particularly the frail, elderly, children, and those with long-term conditions, to navigate their care and access necessary services. This position includes collaboration with GPs, practice teams, social prescribing link workers, and health coaches to create personalized care plans that address patients' changing needs and improve their overall quality of life.

Candidates should possess excellent interpersonal and communication skills, a patient and empathetic demeanor, and prior experience in health or social care roles. Strong organisational skills, a proactive attitude, and the ability to work effectively in a challenging environment are essential. The Care Coordinator will be integral to the PCN's efforts in promoting personalized care, supporting patients in managing their health and well-being, and ensuring they receive comprehensive support from available resources. This non-clinical position may require remote work as needed.
