The practitioner will be responsible for conducting history taking, clinical assessments (both remote and face-to-face), making diagnoses, and creating treatment plans, including prescribing medications where applicable. This role involves interacting with patients from various medical needs, particularly those referred through 111 and 999 services, and offering exemplary care that addresses the specific health requirements of vulnerable and marginalized groups.
