The 3Sixty Care Partnership is a unique joint venture between a federation of 27 GP practices in Kettering, Corby, Wellingborough and East Northants and Northamptonshire Healthcare NHS Foundation Trust. 3Sixty are working together with the five Primary Care Networks (PCNs), by bringing together health and care professionals to support them to deliver joined-up services, which we believe will lead to better quality care, better patient satisfaction and better outcomes.

Northamptonshire Healthcare NHS Foundation Trust (NHFT) on behalf of 3Sixty Care Partnership, and its member practices are excited to announce that they are seeking to recruit motivated and enthusiastic Clinical Pharmacists, with excellent communication skills to join our forward-thinking team to support the safe and effective prescribing of medicines and deliver the Primary Care Network Contract Directed Enhanced Service (DES).

The roles involve working in a multi-disciplinary team in a patient-facing role as well as providing primary support to general practice staff with regards to prescription and medication queries. The successful candidates will take responsibility for areas of chronic disease management within the practice and undertake structured medication reviews to proactively manage patients with complex polypharmacy. In addition, the clinical pharmacist will provide support with medicines optimisation, medicines safety, quality improvement, staff training and manage some aspects of the quality and outcomes framework and enhanced services.

Interested applicants will need to have completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council, as well as experience of working in a pharmacy setting. Applicants will also need to demonstrate a commitment to learning and development and an ability to adapt to different working environments.

We welcome applications from registered pharmacists working in all sectors of pharmacy. The successful candidates will be enrolled on to the Primary Care Education Pathway and be supported to develop their role to become non-medical prescribers.
