The Reconnect Team Lead role at Midlands Partnership NHS Foundation Trust in Kidderminster involves leading the Reconnect Pathway within the expanding Health & Justice Service. The position requires travel throughout the West Mercia area and includes managing a team responsible for conducting assessments for individuals in prison up to 12 weeks prior to their release. The role focuses on providing care for patients with various health vulnerabilities, such as mental health issues, learning disabilities, and physical health concerns.

Applicants must possess strong risk assessment skills and a proactive attitude, ensuring high-quality clinical services while supervising team members, managing complex assessments, and liaising with criminal justice partners. The role entails maintaining effective clinical governance, conducting robust risk management, and advocating for the needs of diverse populations, including both adults and youths.
