Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
Folly Lane is a friendly and proactive practice looking after a diverse population in the centre of Warrington. We have a strong practice team who work hard to deliver excellent patient care. We are looking to recruit a Practice Nurse to cover 20 hours per week, over a Monday, Wednesday and Friday. The post holder will be responsible for the delivery of general practice to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures and provide nurse leadership as required.
Main duties of the job include:-
Chronic disease management
- CVA and Ischaemic heart disease
- Diabetes preferred, successful applicant will be required to train for this if not already holding certificates
- Ensuring safe systems are in place
- Checking of fridge and emergency equipment
- Travel and routine immunisations
- Assisting emergencies
- A mix of face to face and telephone consultations may be required
- Smears
- Cover for treatment room which may include Hypertension clinic, Phlebotomy and ECGs.

- Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
- Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols
- Work with patients in order to support adherence to prescribed treatments
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support and manage health needs of women presenting for family planning and cervical cytology consultations
- Recognise, assess and refer patients presenting with mental health needs in accordance with the NSF for Mental Health
- Implement and participate in vaccination and immunisation programmes for both adults and children
- Advise, support and administer vaccinations where appropriate for patients travelling abroad
- Promote and deliver evidence-based care for patients presenting with aural conditions
- Meet the needs of patients presenting for opportunistic wound care
- Assessing problems presented opportunistically by patients, dealing with minor illness.
- Able to document Consultations according to NMC guidelines
- Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.
- Perform a holistic assessment of patients attending for cervical cytology smear tests.
- Perform venepuncture according to local guidelines
- Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines
- Take ECGs
- Able to recognise and manage anaphylaxis according to current UK guidelines
- Able to perform Cardio-pulmonary resuscitation according to current UK guidelines
- Ensure infection control guidelines are maintained
- Ability to monitor and manage maintenance of stock and equipment to include refrigeration, steriliser and emergency equipment
- Confidentially of information gained at work must be preserved at all times
- Cover the assistance in the provision of minor surgery when other colleagues not in practice.
