Folly Lane Medical Centre in Warrington seeks a dedicated Practice Nurse for a part-time position (20 hours per week) to deliver comprehensive healthcare to a diverse patient population. The role involves managing patients with long-term conditions, providing preventative health care, and performing various clinical tasks such as chronic disease management, immunisations, and minor treatments. The successful applicant will demonstrate critical thinking and clinical decision-making skills while working autonomously and collaborating with the healthcare team to ensure high-quality patient care.

Key responsibilities include assessing and implementing treatment plans for long-term conditions, providing health education, and offering support for family planning and mental health needs. The nurse will also handle minor medical issues, perform routine procedures like venepuncture and ECGs, and participate in vaccination programs.
