CSH Surrey is seeking a Band 5 Community Nurse to join their Integrated Neighbourhood Teams in Woking, specifically across Central Woking and Sheerwater, North and West Woking, East Woking, and South Woking. This role involves providing home visits and care in residential settings, supporting a diverse patient caseload. The position offers excellent training and development opportunities under the guidance of experienced Senior District Nurses and operates on a 7-day schedule with varied shifts, including weekends and bank holidays. Applicants must have a valid Adult Nursing qualification, and newly qualified nurses are encouraged to apply.

The Community Nurse will participate in patient assessment and care planning, effectively communicating with patients and the multidisciplinary team. Key responsibilities include maintaining accurate patient records, implementing evidence-based care, initiating health promotion activities, and participating in clinical audits and service development initiatives. The role requires accountability for professional actions, compliance with mandatory training, and active engagement in supervision and mentorship opportunities, all while ensuring patient safety and adhering to the Nursing and Midwifery Council guidelines.
