The Learning Disability Nurse at King's Lynn Primary Care Network will play a crucial role in addressing health inequalities for individuals with learning disabilities. This position involves conducting annual health checks and assisting with severe mental illness health checks, focusing on identifying health issues early to improve overall health outcomes for this population. The nurse will utilize SystmOne templates to perform necessary assessments, including measuring vital signs, discussing associated conditions, and providing health education to patients and their families. Additionally, the nurse will enhance collaboration with local agencies to improve service delivery and patient engagement.

The successful candidate must meet the qualifications for an Enhanced Practice Nurse, possessing a nursing degree, NMC registration, and a postgraduate qualification. Key responsibilities include working within a multidisciplinary team, developing and evaluating complex care plans, managing clinical interventions, conducting clinical audits, and possibly supervising junior staff. The role may also involve additional tasks as needed to benefit the PCN and its member practices.
