Practice Nurse
into our practice at County Surgery to assess, plan, develop, implement and evaluate treatment programmes for our patients.

What can we offer you as a Practice Nurse?
- As a Practice Nurse, we can offer you an annual salary range of £18 to £20 per hour depending on your experience, skills and qualifications.
- up to 30 hours - flexibility for these to be spread over three or four days
- Full support from an experienced close-knit clinical team including GPs, ANPs, PNs and HCAs
- Enrolment to the NHS pension scheme
About us
- Develop, implement and embed health promotion and well-being programmes
- Undertake the child vaccination programme
- Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
- Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Provide guidance to patients in the use of prescribed and over-the-counter medications on side effects and dosages
- Oversee the provision of wound care services to patients
- Request pathology services as necessary
- Process pathology results as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure read codes are used effectively by all members of the nursing team
- Develop, implement, embed and participate in well woman clinics
- Develop, implement, embed and participate in well man clinics
- Chaperone patients where necessary
- Assist GPs with minor surgery when required
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognise, assess and refer patients presenting with mental health needs
- Implement vaccination programmes for adults and children and travel
- Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
- Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
- Support the clinical team with all safeguarding matters, in accordance with local and national policies
- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
- Deliver opportunistic health promotion where appropriate
- Managing chronic disease as appropriate
- Home visits as required
- Clinical appointments and reviews as per skills list
- Develop and review audit protocols and process for the practice, effectively utilising the audit cycle
- Develop, review and implement nursing protocols in conjunction with the partners
- Support all members of the nursing team, providing guidance when necessary, conducting appraisals and training needs analyses as required
- Participate in local initiatives to enhance service delivery and patient care
- Support and participate in shared learning within the practice
- Continually review clinical practices, responding to national policies and initiatives where appropriate
- Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA)
- Coordinate the nursing team rota, ensuring sufficient staff are available to meet patient needs
- Lead the planning and implementation of changes within the nursing team, enhancing the provision of services to patients
- Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation
- Act as a clinical leader in the delivery of practice nursing services to patients, ensuring that the needs of the patient are the priority
- Lead the nursing team in the planning and implementation of local guidelines, protocols and standards, and of local projects or initiatives
- Promote the role of the nursing team in the provision of care
- Participate in practice team meetings, delivering the nursing agenda and run nurse department meetings
- Promote a learning environment with a no blame culture
- Lead the QoF strategy and delivery for LTC management, maximising income
- Responsible for managing and maximising CCG, PHE and NHSE driven targets (childhood immunisations, flu campaigns, smears etc)
- Support the development of others in order to maximise staff potential
- Lead others to make realistic self-assessment of their knowledge and skills, challenging any complacency and actions that are not in the interest of the public and/or users of service
- Take responsibility for own development, learning and performance including participating in clinical supervision and acting as a positive role model
- Promote a learning environment for patients, nurses and other health professionals.
- Assist with the planning and implementation and teaching for practice staff including medical students, nurse students, nurses and health care assistants.
