The NHS Sussex ICB is seeking a Senior Practitioner Nursing (Bank) to join their Clinical Bank, focusing on the All Age Continuing Care (AACC) service in Lewes. The role involves delivering a variety of functions within the Continuing Healthcare (CHC) framework, including assessments, reviews, care planning, and case management. Candidates should have experience in health or care, with strong communication skills for effective interaction with patients, families, and external partners. Prior experience in CHC is preferred but not essential, as comprehensive support and training will be provided to develop the necessary expertise.

This position requires a commitment of at least three days a week and entails occasional travel for assessments and reviews.
