Cambridge University Hospitals NHS Foundation Trust is seeking a motivated Frailty Specialist Support Nurse to join their Early Intervention Team (EIT). This proactive multidisciplinary team focuses on identifying and managing frailty in patients aged 75 and older, aiming to initiate Comprehensive Geriatric Assessments and providing admission avoidance strategies. The role involves collaborating with various healthcare professionals to deliver holistic care, conducting specialized assessments, and formulating care plans for patients in both hospital and community settings.

The ideal candidate will possess strong teamwork skills and a commitment to quality improvement while being responsible for high standards of patient assessment, advice, and evaluation.
