Penketh Health Centre in Warrington is seeking an experienced Practice Nurse to join its dynamic nursing team, offering flexible full or part-time hours ranging from 15 to 30. The role involves delivering high-quality patient care, managing specific Quality and Outcomes Framework (QOF) areas, and conducting long-term condition reviews for diseases such as Diabetes and Asthma. The Practice Nurse will be responsible for assessing, implementing, and evaluating individualized health plans, conducting chronic disease management clinics, and engaging in health promotion strategies.

Key responsibilities include administering vaccinations and immunisations, reviewing patients' medication for therapeutic effectiveness, and providing emergency care when necessary. The successful candidate will also collaborate with other health professionals to support patients with long-term conditions, participate in national screening and immunisation programs, and contribute to practice initiatives aimed at maintaining high achievement standards. Furthermore, the nurse will assist with home visits for housebound patients and provide support and guidance to healthcare assistants and junior staff as needed.
