The South Warwickshire GP Federation is seeking a full-time Social Prescribing Link Worker for their Arden Primary Care Network in Leamington Spa. This role involves working within a collaborative Health and Wellbeing team across seven GP practices to provide holistic support and guidance to individuals seeking to improve their health and wellbeing through non-medical community resources. The successful candidate will take referrals from various sources, conduct comprehensive assessments, and create personalized support plans, helping patients access local activities and services while promoting the benefits of social prescribing.
