The Community Triage Nurse at Harrogate and District NHS Foundation Trust will play a vital role within the Northumberland 0-19 service team, focusing on triaging and allocating referrals for children aged 5-19. This position involves coordinating and gathering information for children being referred to the school nursing service, particularly concerning potential neurodevelopmental disorders. The nurse will employ a multiagency integrated approach to ensure referrals are directed to the appropriate services promptly and participate in discussions to determine referral destinations based on established criteria.

Key responsibilities include collaborating with various agencies—including education and mental health services—and communicating with families to promote the development of disadvantaged children. The role also requires supporting the School Nurse in planning and implementing care programs for school-aged children and contributing to comprehensive health needs assessments for clients and communities with additional needs.
