Central London Community Health Trust is seeking a Senior Community Nurse in Finchley to provide high-quality nursing care to patients in their homes and various community settings, including providing rapid response and rehabilitative care on weekends and bank holidays. The role involves maintaining collaborative relationships with staff across the North Division and integrated locality teams, assessing patient referrals, and responding to needs based on urgency.

The Senior Community Nurse will utilize advanced skills to evaluate patients' physical and psychosocial needs, create comprehensive care plans in collaboration with patients, their families, and multidisciplinary teams, and conduct risk assessments to ensure patient safety at home.
