Oaklands Health Centre is seeking to hire an additional Practice Nurse to join its expanding clinical team, which currently includes Advanced Nurse Practitioners, a Paramedic, Specialist Nurses, and Health Care Assistants. The Practice Nurse will be responsible for various aspects of primary care nursing, such as cervical cytology, immunizations, wound care, chronic disease management, and QOF requirements.

Oaklands Health Centre is a dynamic surgery that has developed significantly over the few years. We have played a key role in the Covid response within our area and have a strong and well established clinical workforce.
Our clinical team consists of 5 Advanced Nurse Practitioners, 1 Paramedic, 2 Practice Nurses, 2 Specialist Nurses and 3 Health Care Assistants.
We are looking to expand our team by recruiting an additional Practice Nurse to cover allaspects of primary care nursing including cervical cytology, immunisations, wound care, LTC management and QOF.
Core duties will focus on providing excellent patient centered care whilst supporting the practice in achieving quality indicators and requirements of the GMS contract and any relevant National Service Frameworks. This includes -
JOB PURPOSE
Undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs according to practice policy, protocols etc.
Strive to improve standards of care and be involved in identifying practice population needs.
Chronic Disease management is an essential part of the role as well as the mentoring of our Health Care Assistants.
Knowledge of the GMS contract, e.g. The Quality and Outcomes Framework, LES/DES is a vital element of the post.
