The Central and North West London NHS Foundation Trust is seeking an experienced, compassionate Urgent Community Response Nurse or Paramedic for a Band 6 role in Inner London Boroughs, including Kensington and Chelsea, Westminster, and Hammersmith & Fulham. The successful candidate will provide admission avoidance care in patients' homes and will be responsible for assessing, planning, and delivering healthcare needs while collaborating with a multi-professional team. Experience in community or tertiary services is preferred, and candidates must be able to work independently and manage their schedules flexibly, including evenings, weekends, and public holidays.

Key responsibilities include conducting objective and physical assessments, developing care plans, and ensuring patients are managed safely and effectively. The role requires excellent communication skills to educate and support patients and their carers, as well as adherence to professional standards and ongoing training. The position operates within a seven-day service, and although community experience is not mandatory, a deep understanding of the challenges faced when delivering care in home environments is essential.
