CASELOAD EMPATHY HOLISTIC MENTAL HEALTH MULTI-DISCIPLINARY NHS SOCIAL WORK
The Lincolnshire Partnership NHS Foundation Trust is seeking a Senior Mental Health Practitioner to join a dynamic community mental health team that emphasises innovative, compassionate support for individuals in their own homes. The role involves providing high-quality clinical care, conducting thorough assessments, developing treatment plans, and facilitating discharge plans while managing a diverse caseload. Practitioners will collaborate with various stakeholders, including housing services and third-sector organisations, to cultivate a co-designed rehabilitation service focused on promoting independence for individuals with mental health challenges.

The ideal candidate will have a strong understanding of mental illness, effective communication skills, and experience in implementing person-centered, recovery-focused interventions. Responsibilities include mentoring nursing team members, evaluating risk and outcomes, and supporting both patients and their families through tailored programs. The position requires flexibility to work across a 7-day service and a commitment to high standards of care, demonstrating empathy, leadership, and a collaborative approach within a multi-disciplinary team.
