Job Summary: Social Prescriber - Hayling Island & Emsworth PCN
Hayling Island & Emsworth PCN is seeking a dedicated and compassionate Social Prescriber to join a multi-disciplinary primary care team across three GP practices. The role focuses on empowering individuals with health and social needs—such as chronic conditions, loneliness, or complex social issues—by providing holistic support and connecting them with community resources and services.
Key Responsibilities:
  • Work one-on-one with individuals to create personalized care and support plans.
  • Conduct home visits and support clients in identifying social issues that affect their health, like debt, housing, and unemployment.
  • Collaborate with local agencies, GP practice staff, and stakeholders to facilitate referrals and raise awareness about social prescribing.
