Social Prescriber with Hayling Island & Emsworth PCN in Hayling Island, Hampshire
We are looking for a committed and compassionate Social Prescriber to join our multi-disciplinary primary care team. You will be part of our Primary Care Network Team, working across 3 GP Practices on Hayling Island and Emsworth, Hampshire. You will be required to work across our 3 GP Practices. Social prescribing empowers people to take control of their health and wellbeing, taking an holistic approach and connecting people to community groups and statutory services for practical and emotional support. About us As a link worker you will work directly with individuals who have, for example, long term chronic health conditions or people who are lonely or isolated or with complex social needs. You will help people to focus on what matters to them as identified through shared decision making or a personalised care and support plan, meeting on a one-to-one basis and making home visits where appropriate. You will help people identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities, working with the person, their families and carers, to consider how they can all be supported through social prescribing. Collaborating and being proactive in developing strong links with all local agencies to encourage referrals you will also need to build relationships with key staff in our GP practices and stakeholders across the PCN and wider community. An important part of this role is to raise awareness of social prescribing and how partnership working can reduce pressure on statutory services, improve health outcomes and enable an holistic approach to care. The role requires a highly competent, proficient practitioner, who is able to work autonomously and creatively in a busy and demanding environment. Demonstrates personal accountability, emotional resilience and works well under pressure Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines High level of written and oral communication skills Ability to work flexibly and enthusiastically within a team or on own initiative. Knowledge of and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety Rehabilitation of Offenders Act 1974 This post is subject to an exception order under the provisions of the Rehabilitation of Offenders Act 1974.
