The Discharge Facilitator role with the Community Discharge Assessment Team (CDAT) at Leeds Community Healthcare NHS Trust focuses on preventing avoidable hospital admissions and ensuring a smooth transition from hospital to community care. This multidisciplinary team includes Registered Nurses, Physiotherapists, and Occupational Therapists who work in various settings such as the Emergency Department and acute assessment wards. Facilitators conduct holistic assessments of patients to identify safe discharge opportunities, coordinate with community services, and provide tailored therapy support as needed. They operate a 7-day service, conducting daily patient assessments and engaging in ward meetings to discuss individual care needs.

Discharge Facilitators utilize their clinical skills to collaborate with both healthcare teams and patients' families, facilitating referrals to necessary community resources and services to optimize patient outcomes. The role is dynamic and requires adaptability to meet patient needs alongside a commitment to holistic care and teamwork. Interviews for this position will take place in person at St James University Hospital in Leeds.
