The Multiple Sclerosis Practitioner role at Sussex Community NHS Foundation Trust in Haywards Heath offers an opportunity for a clinical specialist to enhance and maintain services for individuals affected by Multiple Sclerosis (MS) in the Crawley/Mid-Sussex area for a fixed term of 12 months. The practitioner will be responsible for providing advanced clinical care management, assessing individual needs, and implementing care programs to empower patients, their families, and caregivers from the time of diagnosis throughout the disease continuum. The role includes promoting and coordinating research-based practices, liaising with local neurologists and healthcare professionals, and leading initiatives to uphold high care standards while advocating for MS services within the community.

The successful candidate will work autonomously with a caseload, providing support in clinics for follow-up and newly diagnosed patients, while also addressing associated conditions and emotional needs. Collaboration across disciplines will be essential to ensure coordinated care, including participation in multi-professional case reviews and managing treatment strategies for MS. The practitioner is expected to maintain current knowledge of MS management and contribute to the ongoing development of the service and clinical standards, all while fostering relationships with various stakeholders and conducting audits for service evaluation.
