Elysium Healthcare is seeking a Social Worker to join their multidisciplinary team at Moorlands Neurological Centre in Stoke on Trent, where they provide care to adults with acquired brain injuries. The role involves managing a complex caseload, conducting social care and risk assessments, facilitating group activities, and contributing to individual care reviews. Social Workers will also be responsible for handling safeguarding matters, managing internal investigations, and working closely with local authorities throughout the admission and discharge processes.

Candidates should have a Social Work degree and experience working with vulnerable adults, preferably in a similar environment. Referrals to LADO and Local Authority in line with safeguarding and assessments

Educating Hospital staff about relevant safeguarding matters.

Completing a full annual social care reassessment within the CPA format.

Facilitating access to welfare rights, housing, advocacy services and associated support services

Participating in 1:1 supervision

To be successful in this role, you'll need:

A Social Work degree

Experience within similar environment

Experience with working with vulnerable adults

Location: Moorlands Neurological Centre, Lockwood Road, Cheadle, Staffordshire ST10 4QU

Moorlands Neurological Centre is being developed as a neuro behavioural hospital service for men and women over the age of 18 years, who have an acquired brain injury. The service will support and care for people who, because of their injury, have challenging and complex needs that are preventing them from making further progress with their lives and require a neuro behavioural rehabilitation programme
