Aneurin Bevan University Health Board is seeking an Assistant Practitioner for the Integrated Community Resource Team in Tredegar. This role is designed for passionate individuals who want to make a positive impact on the lives of adults requiring support to maintain independence in their own homes. As part of a multidisciplinary team that includes Social Workers, Physiotherapists, and Home Care professionals, the Assistant Practitioner will provide essential clinical interventions, engage in assessment and caseload management, and offer hands-on support to clients in various community settings.

The position emphasises collaboration with community and social care partners to deliver timely interventions aimed at promoting prevention and early support. Candidates should possess strong problem-solving abilities, excellent time management skills, and experience in a supportive role within a multidisciplinary environment. Proficiency in Welsh is desirable, although applications from English and/or Welsh speakers are welcomed. This role is integral to delivering high-quality health and social care services in Blaenau Gwent.
