The role of Community Mental Health Nurse with the East London NHS Foundation Trust involves delivering high-quality, patient-centered care as part of a fixed-term pilot project within the Tower Hamlets Community 24/7 service. This innovative initiative aims to provide accessible, co-produced care that addresses both physical and mental health needs, operating around the clock and ensuring support for individuals in crisis through various psychosocial and medical interventions. The position requires collaboration within a multidisciplinary team to develop and review care plans, conduct comprehensive risk assessments, and ensure that compassion and empathy remain central to service delivery.

The nurse will play a vital role in liaising with service users, local communities, and health and social care providers, focusing on holistic care and promoting the well-being of individuals with co-morbid conditions.
