FAMILY MENTAL HEALTH MULTI-DISCIPLINARY NHS SOCIAL WORK
The Senior Home Treatment Team Practitioner role at Pennine Care NHS FT in Ashton-Under-Lyne involves delivering recovery-focused interventions to adults experiencing a mental health crisis, with the goal of enabling them to remain in their own homes. The ideal candidate will be a qualified Mental Health Nurse, Occupational Therapist, or Social Worker with substantial post-qualification experience, particularly in crisis services. Key responsibilities include conducting comprehensive mental health assessments, managing risk, developing individualized care plans, and collaborating with multidisciplinary team members to provide intensive home treatment and crisis management.

Practitioners will be required to work autonomously within the community, while also integrating closely with other professionals and agencies to ensure effective treatment pathways. This role necessitates confidence in risk assessment and decision-making regarding the appropriateness of home treatment, alongside the application of Pennine Care NHS Trust policies and the Mental Health Act. The position operates within a 24/7 care model, requiring the flexibility to undertake both day and night shifts on a rotational basis.
