The post-holder carries continuing responsibility throughout the 24-hour period for the assessment of care needs, the development of programs of care, and the setting of standards of care. Also carries continuing responsibility for the management of the ward, including deployment of nursing staff and teaching of learners.
Agree with the DDoN, HoN and Modern Matron the standard of nursing care and ensure that these are maintained at optimum levels.
Co-ordinate the provision of patient care throughout the 24 hour period, ensuring that there is effective communication within the ward, liaison with medical staff and other disciplines and services.
Be responsible for the assessment of patients' care needs and the development, implementation and evaluation of research based programmes of care, to ensure that these comply with all policies and guidelines as laid down by the Dartford & Gravesham NHS Trust and all legal/statutory requirements.
Ensure that all nursing records and documents are completed accurately and legibly by self and team, in order to meet the Dartford & Gravesham NHS Trust and legal requirements.
Ensure that all staff are up to date with their competencies

Individual care plan/treatment records
Delivery of care
Where treatment occurs different wards/settings
Liaison and report writing
Out of hours service responsibility
Involvement of patients and carers in care plan
Continuity or care and progression of care
Development of services
Standards of care and evidence based practice implementation and audit
