The Great Western Hospitals NHS Foundation Trust is seeking a compassionate and dedicated Health Care Support Worker to join the Swindon End of Life Care Team (SELeCT). In this role, you will play a vital part in facilitating rapid hospital discharges and providing emotional and personal care to patients in their final days at home. Candidates should have a strong enthusiasm for community nursing, particularly in end-of-life care, as well as the adaptability to learn in a dynamic environment.

The position requires excellent communication skills, empathy, and a kind demeanor to support patients and their families during difficult times. Shifts may include evenings and weekends, and you will need a driver's license and access to a vehicle. Familiarity with electronic record-keeping is essential, and new team members will be expected to complete Care Certificate training if not already qualified. The SELeCT team is committed to providing holistic support and care for end-of-life patients throughout the year.
